cified peritoneum. Diagnosis of EPS was made. Steroids (0.5 mg/kg/day) and Tamoxifen 20 mg twice per day were started. The patient is now on a normal diet and CRP has returned to normal. A definitive diagnosis of EPS is done at laparotomy or laparoscopy but may be strongly supported by a combination of clinical and radiologic features [1] . Computed tomography has largely been used to delineate more advanced disease. Abdominal X-ray is not useful but in our case, peritoneal calcification is prominent. Risk factors are duration of peritoneal dialysis, peritonitis, poor biocompatibility Encapsulating peritoneal sclerosis (EPS) is a rare complication of peritoneal dialysis and has an incidence rate of 1.8/1,000 patient-years [1] . The usual clinical presentation is with partial or complete small bowel obstruction, ascites, abdominal mass, or impaired peritoneal ultrafiltration [2] . We report the case of a patient with EPS according to the original clinical and radiological presentation. A 41-year-old man was admitted on December 29, 2009, for diarrhea and highgrade fever (40 ° C). He began peritoneal dialysis in May 2001 for end-stage renal disease. He experienced three episodes of peritonitis which improved after treatment. The patient was transferred to hemodialysis therapy because of poor urea clearance and ultrafiltration on November 23, 2009. Examination revealed a distended abdomen, hypoactive bowel sounds, and diffuse abdominal tenderness. Investigations revealed no abnormality except CRP 300 mg/l and elevated serum creatinine. Abdominal radiography was performed ( fig. 1 ) and showed extensive peritoneal curvilinear ‘eggshell’-like calcification associated with vascular calcifications. The diagnosis of EPS was hardly suspected. Abdominal computed tomography showed dilated bowel loops with thickened walls, diffuse parietal and visceral peritoneal calcification, peritoneal thickening and enhancement, and encasement of the dilated bowel loops by the calPublished online: October 6, 2010

Case report: acute bowel obstruction with an isolated transition point in peritoneal dialysis patients; a presentation of encapsulating peritoneal sclerosis?
BACKGROUND Encapsulating peritoneal sclerosis (EPS) is classically described as progressive sclerosis and cocooning of the entire peritoneum; however, there has been limited number of reported cases of localized fibrosis as a variant form. CASE PRESENTATION We describe two cases of acute bowel obstruction with isolated transition points in the setting of long-term peritoneal dialysis. CONCL...
متن کاملEncapsulating peritoneal sclerosis--a complication of peritoneal dialysis.
Encapsulating peritoneal sclerosis (EPS) is a rare complication of peritoneal dialysis. It often presents with non-specific symptoms, leading to a delay in diagnosis and a poor prognosis. Here we report a case of EPS in a patient treated with peritoneal dialysis and discuss the risk factors, diagnostic challenges and treatment options available.
متن کاملCase Report of a Patient Undergoing Peritoneal Dialysis with Encapsulating Peritoneal Sclerosis Superimposed With Calciphylaxis
INTRODUCTION Encapsulating peritoneal sclerosis (EPS) is a rare but devastating complication of peritoneal dialysis (PD). Tamoxifen has been generally well-tolerated, even without randomized controlled trials. CASE PRESENTATION Herein, we report a case of a patient undergoing 12 years of PD who developed EPS and calciphylaxis simultaneously. We also provide a comprehensive discussion about th...
متن کاملSuccessful outcome with early renal transplantation following treatment of encapsulating peritoneal sclerosis
Encapsulating peritoneal sclerosis (EPS) is a rare complication of peritoneal dialysis (PD) associated with cocooning of the bowel within a sheath of fibrous tissue causing obstruction. Uncertainty remains regarding the aetiology, pathogenesis and optimal treatment. The current management of EPS is largely based on case reports and small series, and there are limited data regarding outcomes of ...
متن کاملA Case of Encapsulating Peritoneal Sclerosis Complicated by Malignant Peritoneal Mesothelioma.
We report a case of peritoneal mesothelioma discovered in a patient during peritoneal dialysis. The patient was a 55-year-old woman who had no history of asbestos exposure. Owing to end-stage kidney failure, she had been undergoing peritoneal dialysis for over 8 years, and she had been diagnosed with encapsulating peritoneal sclerosis. She was admitted to the hospital for intestinal obstruction...
